I waited over a week to receive my call and find out I got an offer. Sometimes it just takes time for the interviewers to get on the line together and take a decision.

You still have a chance. There could be several reasons for the delay:
It is fine to follow up again after one week (I would call rather than send an email).

Waiting for the outcome is arguably the worst part of the process and I really feel with you! However, nothing is lost nor won based on the time that has passed.
As for your questions:

Normally partners need time to meet up and discuss your feedback. If your interview was late on Friday they will have pushed it back either on Monday or today. A friend of mine had to wait a full working day to get his BCG offer.
If it gets to Wednesday evening and you still haven't heard back, that is not a great sign.
No point in worrying about it now. I know my words won't be of solace but be proud you made it so far! Not many do.

MBB is generally good at giving the decision as soon as they have it. Usually people that get offers get told first and rejections afterwards but they are not that far apart.
The reasons for delay are almost always related to logistics of getting all the interviewers together in one room. Sometimes its about finding the time to schedule a call to inform you (e.g., people could be away on vacation)

That is still less than one week and within the normal/average bounds.
I would not worry about it at this point as interviewers need to align/meet to discuss their experience, HR needs to be informed, etc. There are a lot of moving parts in this!

It can take weeks until you get an answer… so hand in there and be prepared to wait. Waiting means… nothing in itself, other than that they don't have an answer yet.
