Sometimes it can take up to two weeks.
I wouldn't follow-up after 3-4 days. Give them at least 10 days.
I know it's difficult to be waiting around (I also waited for 10 days after my McK interview), but rest assured, they didn't forget about you.
If it helps you, keep on applying to other firms. This way you can at least diversify some of your risk.

After a full week it's more than fair to follow up for MBB as their processes are more organized. Don't worry too much coming across too eager. Won't change the rating of interviewers ;-)

Depends on the purpose of the follow up.
Thank you note: Can be later the same day or next day (no later than that). It's optional but generally well received. Impact is none-marginal and may help tip the scale where it's really 50:50.
Enquiry about status: Don't bother until 2 weeks after the interview if you don't hear anything. This can take a while and there's no point in pushing. Impact is 0, other than for you to manage anxiety during the wait.

I would follow up typically a week or maximum 2 weeks after the final round. When I was at BCG, we tried to get back to candidates within 1-2 weeks max.

Generally, following up is more for your peace of mind rather than actually having any impact on the process (you can't change anything just from a follow-up).
I wouldn't follow-up in 3-4 days (too pushy and you “waste” a future reminder).
