Theoretically, you should have received a formal email if they rejected you.
I would reach out to the recruiter if I were you to look into it.
Aside from this, I'd recommend you apply to other firms as well. Having a broad application strategy is one of the success factors of getting an offer, as I also outlined in the guide below:

It most likely does mean you have been rejected.
In this case you can apply again in about 10 months. When you do, make sure you have improved your resume and networked extensively.
Make sure to apply to the other dozens upon dozens of great consultancies out there!

Most likely yes, but they should also send a formal rejection email in that case. If you want you can contact HR to check the status.

The usual ban is 12-18 months. Normally they send a rejection email to clarify that, if you don’t receive one you can also reach out to HR.

from my experience in McKinsey, you should always receive an email with formal feedback from the Firm - check your spam to make sure you haven't lost it! If it's not there, I'd suggest following up with HR.
However, I'm afraid it's a bad sign as very often the "good news" is given with a phone call.
But don't beat yourself up. I know several examples (including friends and colleagues) who were rejected the first time and managed to get an offer later after preparing to ace the interviews.
Normally, you have to wait at least 12 months before applying again. I'd suggest using that time to develop a solid preparation plan with a clear roadmap.

Unfortunately, this does most likely mean that you got rejected. However, it's odd that you haven't received an email to confirm this. I would reach out to HR to understand the status of your application. If you have been rejected, you usually cannot reapply for a period of between 12-18 months. The precise length of the ban would have been provided in the rejection email, so HR should also be able to provide you with this information.
