Is there a very small chance of error? Sure but in my experience that is almost never heard of these days. If you feel something is definitely wrong or contradicts what you were told by HR, you can reach out to the recruiters for an update.

Statistically speaking yes, there is a chance of error as the status is manually set by Recruiting, the margins are small though. I wouldn't make bets on it.
