I hear in some Bain offices there is only 1 interview in round 1 but in others there are 2 interviews. In the case of 2 interviews, do you need a yes from both or if one is yes and other is no, can you still pass?

You need to have a solid performance in all interviews. In situations where one interviewer is on the fence (a maybe) while the other is a strong yes, you might be able to get away with passing onto the next round if there is availability, and there may be a few other edge-case scenarios as other experts here have pointed out. Particularly in the later rounds you definitely need all interviewers to feel positive about your performance. A ‘no’ from an interviewer will usually arise due to specific concerns, which wouldn't just be overridden by a good performance in another interview.

You need to demonstrate a consistently strong performance across all interviews to move to the second round.
If there are doubts, there are two possible outcomes:
- You get rejected
- You move to the 2nd round with question marks and the 2nd round interviewers would focus more on the perceived areas of weakness
A third decision interview (as Francesco stated) is not common in the first round and is usually only done after the 2nd round.

If you have two interviews, you need that both interviewers agree your performance was good enough to pass.
If you get a yes and a maybe (meaning it is not a clear pass but it also not a clear no), some companies allow to have an extra interview to check the question marks on your performance.

Consistency is really really important here, so make sure you get a wide variety of cases/scenarios when preparing for interviews!
