Hey,
this is a tough question that cannot be generally answered!
I would talk to people who work in the London Office vs. the Milan office and ask for their experience (how is the infrastructure at the office, community, staffing, size of office!, Italian working style vs. English).
Then it depends on your preferences & also the place you like to live in!

Thanks for your replies!
Maybe I have explained badly what I meant.
My question was not around place's culture, weather, food etc. But more on which are the differences in terms of office itself. E.g which are the focused practice for both offices, what type of projects does each office work more on? How the work-life balance differentiate the two offices?
